Investigate An Incident
After the incident investigation is complete and the findings are known open the incident in IFS and go to the Cause Analysis tab. Here you can select the Incident Causes dropdown to select one or more causes from the list as well as enter responses to the 5 Whys and the Root Cause.
Next go to the Recommendation tab
Here you can enter a summary of the corrective action as well as enter any individual action items to be take.
Incident Correct Action – This an optional summary of the correction action taken for this incident
Safety Actions Required Fields
Safety Action – A brief description of the action item
Target Completion – Target completion date for the item.
Create WO – If this box is checked a work order will be created
Org Code – Org Code to be used when creating the work order. Only required if the Create WO is checked. To select the org code place your cursor in the field and press F8 or click the list of values icon from the toolbar.
If you choose to create a work order for the safety action(s) the work orders will be created once you hit the save button and you’ll see the work order number along with the work order status populate in the respective columns. These will stay updated as the status of the work order changes. Also, when the work order is closed the system will also update the respective safety action on the incident to complete.
Attaching pictures and documents
To attach pictures and documents to the incident go to the General tab and open the Attachments tab at the bottom of the screen. For pictures it’s best to drag/drop them onto the media panel as this an easier method (fewer steps) than attaching a document and this is where the reporting piece will be looking for attachments when the final investigation report is generated.
After the incident investigation has been reviewed, all information from the investigation has been entered and any pictures attached click the “Incident Reviewed” checkbox on the General tab to verify that the review has been done and click Save. Next, right click in the header area of the screen and select the Investigate option. This will generate the final investigation report (similar to what we manually send out now) and send to whatever email distribution group(s) we choose.
If you attempt to set the incident to “Investigated” before checking the Investigation Reviewed box and saving, you will receive the following message.
